SUBJECT : SURVEY QUESTIONNAIRE RE: COMPREHENSIVE

EVALUATION OF DISTANCE EDUCATION IN THE CITY
SCHOOLS DIVISION OF DASMARIAS

DATE : May 27, 2021

In view of distance education that our Department has employed so that learning continues amidst
Covid-19 pandemic, this Office through the School Management Monitoring and Evaluation Section
under the School Governance and Operations Division will conduct a purposive study titled,
"Comprehensive Evaluation of Distance Education in the City Schools Division of Dasmariflas"
which aims to capture insights and feedback from the school administrators, teachers, parents and
learners regarding the implementation of distance learning in our Division and to gather information to
be used in preparing a valid, relevant and reliable plan addressing the gaps in distance learning.

The respondents of the study are the Public Elementary and Secondary School Principals, Public
Elementary and Secondary School Teachers, Parents and Learners. They can access the survey
questionnaire using the link: https://tinyurl.corn/DistanceEducationDasma on or before June 4, 2021.
