We, the undersigned of Grade 11-Zeus students, are conducting a research study
entitled “A QUALITATIVE RESEARCH ABOUT THE SAFETY AND HYGIENIC
PRACTICES OF STREET FOOD VENDING NEAR AND WITHIN THE VICINITY OF
CATAGGAMAN NATIONAL HIGH SCHOOL”. This study aims to determine and
assure the satisfaction of needs in terms of the safety and hygienic practices of
vendors, and the awareness and assessment of consumer students regarding food
safety. In this connection, we respectfully seek approval from your office to allow us to
float our questionnaire to our identified respondents.

We believe this study's result will provide additional information for students to better
understand the importance of basic food safety and hygiene knowledge. Moreover, the
results of this study will serve as baseline data for the school to create and ensure a
peaceful and safe environment for every student of Cataggaman National High School.
