 Design intent description – including statement of the intended operating range


(envelope)
 Find out possible hazard and operational problems – consider possible DEVIATIONs from
the design intent
 Suggest the causes for the relevant deviation
 Estimate the consequences – safety, environment, availability
 List the existing safeguards
 Assess the risk rate
 Suggest additional safeguards based on the risk level (non-tolerable risk level)

 This simple example shows how a HAZOP study works.


 Consider filling the fuel tank .
 Assume the design of the filling the fuel tank / tanker unloading system is
complete.
 List the stage of the tanker unloading process.
 List the steps of each stage.
 Choose one of the stages.
 Describe its Design intent.
 List the parameters.
 Suggest possible deviations from the design intent.
