S A male patient named Malik Hassan Abd El-Daiem aged 2 years and 4

months came to PHC with his mother to show concern about the child
that he developed sore throat and cough since 3 days ago, the
condition started with acute onset progressive course, cough is dry
and no relieving factors, these symptoms associated with rhinitis and
voice hoarseness, itchy skin, and mild fever. The child isn’t on any
medications currently and hadn’t taken any, he doesn’t have allergy
of any food or medications no history of hospitalization, operation,
blood transfusion. No family history of chronic diseases, twins,
congenital anomalies, and negative parents' consanguinity. The child
lives in a 3-room apartment with his 2 siblings and parents, with good
water supply and sewage. Review of other systems is irrelevant.

O vital signs & measurements: The child looks well, No pallor, no


cyanosis nor jaundice, no enlarged lymph nodes, no lower limb
edema. RR: 27 Breath Per Minute, Pulse: 88 Beat Per Minute.
Temperature: 38.20 C (low grade fever), Weight: 13 kg, Height: 92 cm,
Head circumference: 48 cm, Mild throat redness, presence of small
ulcers on posterior pharynx. No congested tonsils, No ear pain or
discharge, no lymphadenopathy. Chest examination was free of any
positive findings.
No lab Investigation were done:

A Diagnosis: Acute viral pharyngitis.

He needs to be treated symptomatically. Pain is best treated with

P paracetamol or ibuprofen for pain, Dermatop cream (Corticosteroid,


anti-inflammatory) for itchy skin, Loratadine (Anti-histamine) for
rhinorrhea. Also, home remedy such as hot drinks and soup is
important. Follow up in 5 days if no improvement. Return immediately
if child not able to drink, becomes sicker, develops fever.
