Generic name: amoxicillin Dosage: 500mg/cap Frequency: q8 Action: Prevents bacterial call wall synthesis during replication. Bactericidal antibiotic Indication: Treatment of infection of respiratory skin and skin structures genitourinarytract,otitismedia,meningitis,septicemia,sinusitis,bacterial,endocarditisproph ylaxis. Nursing responsibilities: culture infected area prior to treatment; re-culture area if response is not as expected. Give in oral preparations only, amoxicillin is no affected by food. Continue therapy for at least 2 days after signs of infection have disappeared continuation for 10 full days is recommended. Use corticosteroids or antihistamines for skin reaction. Advise patient to watch out for and report signs of super infection or agranulocytopenia like vaginal itching, loose foul smelling stools or furry tongue. Teach patient to report sore throat, bruising, bleeding, joint pain and bloody diarrhea which may indicate dispraises (rare) and pseudomembranouscolitis respectively.May give with food to prevent GI upset, store in tight container. Indication: Relief of pain including muscular rheumatic,traumatic,dental postoperative post partum partum pain , headache,migraine,fever anddysmenorrheal, pain from rheumatoid arthritis(RA)including Stills Disease, soft tissues injuries. Therapy should not exceed 7 days. Nursing responsibilities:Assess patients who develop severe diarrhea and vomiting for dehydrationandelectrolyteimbalance.Lab tests:With long-term therapy (notrecommended) obtainperiodiccompletebloodcounts,HctandHgb,andkidney functiontests.Discontinue drug promptly if diarrhea,darkstools,hematemesis,ecchymoses,epistaxis, or rash occur and do not use again. Contact physician. Notify physician if persistent GI discomfort, sore throat, fever, or malaise occur. Do not drive or engage in potentially hazardous activities until response to drug is known. It may cause dizziness and drowsiness. Monitor blood glucose for loss of glycerin control if diabetic. Do not breast feed while taking this drug without consulting physician. Generic name: DiclofenacDosage: 50mgFrequency:q8 Inhibits cyclooxygenase (COX), an enzyme needed for the biosynthesis of prostaglandin, subsequent decrease in prostaglandin result to the analgesic, antipyretic and antiinflammatory effects. It is use mainly as the sodium salt for the relief of pain and inflammation in the various conditions muscular skeletal and joint disorders such as rheumatoid arthritis ,Osseo arthritis ,and an kylosingspondylitis;peri- particular disorders such bursitis and tendonitis; soft tissue disorders such as sprains and strains; and other painful conditions such as renal colic, acute out, dysmenorrheal, and following some surgical procedures.Ophthalmic solution. Teach patient to avoid use in 23rd trimester of pregnancy.

Advise patient to get emergency medical help if you have chest pain, weakness, shortness of breath, slurred speech, or problems with vision or balance. Tell patient that diclofenac may also cause serious effects on the stomach or intestines, including bleeding or perforation (forming of a hole). These conditions can be fatal and can occur without warning while you are taking diclofenac, especially in older adults. Teach patient to take full glass of water to enhance absorption and to remain upright for hour after dose. If dose is missed, take as soon as remembered within 2hours if taking dose of 1-2x/day. Do not double doses. Call your doctor at once if you have symptoms of stomach bleeding such as black, bloody, or tarry stools, or coughing up blood or vomit that look slice coffee grounds. Remind patient to ask a doctor or pharmacist before using any cold, allergy, or other pain medicine. Medicines. Similar to diclofenac are contained in many combination medicines. Taking certain products together can cause you to get too much of this type of medication. Check the label to see if a medicine contains aspirin, ibuprofen, fetoprotein, or naproxen. Avoid drinking alcohol. It may increase your risk of stomach bleeding. Nursing responsibilities: Tell patient to monitor for and report drug induced adverse reactions but not to stop taking the drug abruptly because withdrawal symptoms (anxiety, increased BP, headache, insomnia, increased pulse, tremors, nausea and sweating) may occur. Teach patient to comply with dosage schedule even if feeling better. Remind patient that the drug does not cure the disease but only controls the symptoms. Tell patient to take weight daily and to report weight gain more than 5 pounds. Diuretics can relieve sodium and water retention. Warn patient that drug may impair mental alertness (dizziness, fainting, lightheadedness) particular at the start of therapy. Avoid hazardous activities and change position slowly (rise slowly to sitting and standing position) to minimize orthostatic hypotension. Tell patient to monitor Blood pressure regularly. Inform patient that excessive perspiration, dehydration, vomiting and diarrhea may lead to fall in BP. Tell patient that dry mouth can be relieved by hard candy, ice chips or sugarless gums. Give between meals with water but may give with meals if gastrointestinal discomfort occurs. Transient staining of mucous membranes and teeth will occur with liquid iron preparation. To avoid, place liquid on the back of the tongue with dropper or use straw. Avoid simultaneous administration of antacids or tetracycline Do not crush sustainedrelease preparations. Eggs and milk inhibit absorption.

Monitor serum iron, total iron-binding capacity, reticulocyte count, hemoglobin, and ferrite.Monitor daily pattern of bowel activity and stool consistency. Assess for clinical improvement, record of relief of symptoms (fatigue, irritability, pallor, parenthesis, and headache). Tell the patient that stools will be darkening in color. If gastrointestinal discomfort occurs, take after meals or with food. Do not take within 2hours of antacids because it prevents absorption.
