Labral Tears

3 structures:
- biceps long head tendon
- glenoid labrum
- glenoid fossa
- head of the humerus
- delts
- SITS

Common MOI/ risk factors:


- repetitive overhead activities (eg. swimming, throwing sports & weightlifters
- anterior shoulder dislocation
- MVA
- FOOSH

3 signs / symptoms:
- sensation of locking, popping, tearing or grinding in shoulder
- sharp pain in shoulder
- decreased GH AROM FLX & ABD
- pain with lifting objects overhead

1 diagnosis/ special test:


- clunk test
- biceps tension test
- whipple
- active compression of O’Briens

1 differential or relieving test:


- speeds test (to rule out bicipital tendinitis)
- drop arm test (to rule out rotator cuff tear)

2 tx goals/ modalities:
- increase capsular space in GH -> distraction of the GH (use caution w/ joint mobs, do
not put GH in painful position or cause further damage to the labrum
- decompress muscles attached to labrum (biceps long head, also should treat SITS &
delts)
- decrease HT biceps long head -> TrPR, mm stripping, compressions

1 strength:
- serratus, UT, SITS

1 stretch:
- genie stretch (place arm across body medially rotated, with opposite hand grab the top
of the elbow and gently pull arm across body. This is to create posterior joint space)

1 ddx condition:
- bicipital tendinitis
- RTC tear
- impingement
