    1. I am a student applicant of Northwest Samar State University (NwSSU), Rueda Ext., Brgy. Balud, Calbayog City,
    Samar, Philippines;

    2. I understand that if I will be allowed to enroll this semester, I will be ON PROBATION and I promise to submit
    the original copies of the Admission Requirements listed below before enrollment the following semester, to wit:

    [ ] Honorable Dismissal/Transfer Credentials


    [ ] Official Transcript of Records (For further studies)
    [ ] Certificate of Good Moral Character
    [ ] Certificate of Live Birth issued by PSA
    [ ] Certificate of Marriage Contract issued by PSA
    [ ] 2”x 2” ID Picture (White Background + Name tag)

    3. I understand that my enrollment is provisional and that the University reserves the right to initiate action
    against me, including filing criminal charges, if any information I have provided or documents submitted are
    found to be fraudulent, fabricated, or falsified.

    4. I understand that I WILL NOT BE ALLOWED TO ENROLL AFTER THE TWO (2) SEMESTERS if I am not able to
    comply the required admission requirements.

    5. I further promise to WAIVE MY RIGHT TO RECEIVE AND DEMAND CREDITS and/or DOCUMENTS for work
    done at Northwest Samar State University, Calbayog City, until I can fully satisfy the above-mentioned
    deficiencies.

    6. I am voluntarily executing the foregoing UNDERTAKING / WAIVER in the order for the University to proceed
    with my enrolment and I fully understood the contents thereof;
