With reference to the discussions regarding claim of above mentioned vehicle, I Mr. AMIT PARAMANIK 
agree to receive a sum of Rs. 2,77,651/- (Rupees Two Lakh Seventy-Seven Thousand Six Hundred Fifty-
One Only) in full and final settlement of my claim on Constructive Total Loss basis.

By virtue of this settlement, I/we discharge the company of their liability & shall have no other claim
whatsoever against the Insurance Company with regard to this claim.

I hereby agree to surrender the captioned Policy & Insurance Certificate for Cancellation without any
refund of premium. It is understood that this settlement will be subject to satisfaction by the authorities
concerned. I/we also understand that this settlement is subject to compliance with all the provisions of
Motor Vehicles Act, 1988 and the Rules made thereof.

I/We undertake to provide any further information or documents, which may be necessary to finalise
the claim.

I/We hereby authorize that the payment shall be made & released in favor of Mr. AMIT PARAMANIK  /
Financier as applicable.
