This is to certify that the sampling collection for the proposed research project of Faith
Nicole V. Agudo titled, “Molecular Identification of Echinostomes (Trematoda:
Echinostomatidae) of Rats from Selected Areas in the Philippines” to be conducted in
Tres de Mayo, Digos City has been presented to this office.

This is to certify further that this office interpose no objection whatsoever to the conduct
of this research project provided that a copy of the results of the study shall be furnished
immediately after its conduct.

This Prior Informed Consent Certificate (PICC) is issued to support the Masters
