Performed daily site inspection, found unsafe condition
the webbing sling used to lift the wiremesh material
was damaged, potentially causing the webbing sling to
cut off and the material to fall down
Location: Corporate Office (driveway)

Performed daily site inspection, found unsafe condition
the bolts on the bar cutter blade were loose and also the
bar cutter blade was damaged

PIBK workers who were painting the cable ladder
hangers did not wear special PPE (safety goggles,
chemical masks, and safety gloves) Location: Ramp

PIBK workers who were doing hot work (welding) did
not wear special PPE (welding mask/caps, welding

Performed daily site inspection, found unsafe action Workers have been given warnings regarding
PIBK workers who were doing wall plastering work did the use of standard PPE when working at
not wear special PPE (full-body harness) when working heights.
on scaffolding, potentially causing workers to fall down
and get serious injuries
