On December 20th, 2022 time 09:00 AM while BVI-
HSE Performed daily site inspection, found unsafe Work was temporarily stopped, because the
action PIBK workers who were doing steel painting scaffolding material installed was incomplete
work using scaffolding that was not proper and not and the scaffolding was tagged red.

condition the electrical panel casing was damaged and panel is equipped with safety signs and PIC
there was also no safety sign, PIC, and single- line
diagram posted on the electrical panel

On December 20th, 2022 time 09:12 AM while BVI-
HSE Performed daily site inspection, found unsafe Work stopped, until basket Oxygen and gas

causing gas cylinders to collapse and explode
