More Information – Dental cover

It is important that you read the Provider’s Employee Guide for details regarding the benefits. The Employee
Guide supersedes all other documents.

Do I have to change my dentist?

No. Dental Cover allows you to visit any dentist you choose – worldwide.

Are there age restrictions for this benefit?

There's no age restriction for you or your partner.

Cover for children continues until they turn 21 or 24 if in full-time education.

What level of charges are reimbursed?

Dental fees vary from dentist to dentist. To make sure you select that plan that’s right for you, we
recommend comparing your dentist’s charges to the cover available.

• NHS treatment: all plans cover 100% of any NHS treatment charges you incur, up to the annual
maximum listed on the schedule.
• Private treatment: the plans cover up to the fixed amount listed on the schedule.

Note that children who visit an NHS dentist receive free NHS dental treatment if they’re under 18 (19 if in
full-time education) at the start of the Plan Year.

How do I make a claim?

You can visit any dentist you like and there’s no need to gain prior approval before starting treatment
(unless you’re claiming for a veneer) but if you’d like to check how much you’re entitled to claim please
contact Unum Dental.

There’s no need to use paper forms with your policy you will have access to Unum Dental’s online portal to
make claiming quick and easy, you can even use your smartphone.

1. Ask for an itemised receipt from your dentist which contains a full description of your treatment
and costs
2. Visit https://mypolicy.unum.co.uk and login using your Unum Dental username and password
(registration is required to begin)
3. Select ‘Make a claim’ on the portal, enter your treatment details, upload a scan or photo of your
receipt and hit submit.

Please contact Unum Dental if you wish to submit a claim via a paper form.
Please note:

• To ensure your claim is processed as quickly as possible, please include details of your treatment,
your dentist's details and proof of payment
• We cannot process any claim without proof of payment.

Claims should be submitted within 90 days of the completion of your last treatment in any course. Unum
Dental reserve the right not to any claims submitted after 90 days.

Can I claim for a pre-existing condition?

Yes. Pre-existing dental problems are covered – aside from mouth cancer. Note that treatment received
before your cover starts or after your cover ends is excluded from Dental cover. In addition, cosmetic
treatments such as tooth whitening aren’t covered.

When can I start making claims?

From the day your policy starts.

When does cover start and end?

Cover starts one month after finalising your benefit selections (take note of the monthly benefit selection
cut-off date). It’ll continue until the annual scheme renewal date.

What happens at renewal?

You’ll have the opportunity to amend your selection ahead of the annual renewal date. If you don’t make
any changes ahead of renewal, the cover will automatically renew for a successive twelve-month period on
the new rates and terms that apply from renewal.

Can I cancel or make changes?

Yes. Changes may be made prior to renewal if you have a qualifying life event.

How is the premium paid?

Your monthly salary will be reduced based on the cost of the benefit, and the company will pay the
premiums to the insurer.

What happens if I leave the company?

Your cover concludes at the end of the month that you leave the company. There’s a full month’s reduction
for the month that you leave to reflect a full month’s benefit cost.

To keep your dental cover, contact Unum Dental within 30 days of leaving for continuation options.
What are the tax and National Insurance implications for this benefit?

Dental cover is treated as a benefit-in-kind. This means you’ll pay income tax on the cost of providing this
benefit. However, this cost is not currently subject to employee National Insurance contributions.

Premiums are inclusive of Insurance Premium Tax at the current rate.

What effect does the reduction in salary, to cover the cost of this benefit, have on other pay-related
benefits?

Other pay-related benefits will continue to be based on your salary before it’s reduced for the benefits that
you select.

What happens next after I select the benefit?

Shortly after the policy start date Unum Dental will email you your welcome email as well as your Policy
Schedule.
