called "PHILHEALTH")

and Industry (DTI) and existing under the laws of the Republic of the Philippines 'Nith business address at
Brgy. IX, Lucena City, represented herem by its Proprietor, ELSIE LIM TAN (herein::~fter called "LC
DIAGNOSTIC'}

WHEREAS, PHILHEALTH invited Bids for the Procurement of Periodic Health

Examination for PRO IVA Employees for CY 2021 and ha s accepted the Bid by LC DIAGNOSTIC, for
the provision of said services in the sum of THREE HUNDRED FIFTY EiqHT THOUSAND SIX
w HUNDRED SEVENTY FIVE PESOS ONLY W 358,675.00) (hereinafter called "Contract Price'').

NOW, THEREFORE, for and in consideration of the foregoing premises, th e parties hereto have
agreed a~ they hereby agree and bind themselves as follows:

In this Contract, words and expressions shall have the same meaning as are respectively assigned to

3. In consideration o f th e payments to be made by PHILHEALTH to LC DIAGNOSTIC as

v 4. PHILI-IEALTH hereby covenants to pay LC DIAGNOSTIC in considera tion o f the prm-ision of

;::: the goods and services and the remedying of defects therein. the Contract Price at the time and in

5. The contract price covers the costs of all Deliverables Items and Services and mclud es all applicable
taxes, including the 12°/o Value-Added-Tax, customs duties, License fees freight, insurance, cost o f
importation, transportation and deliven- at the time and locations specified and other charges which
may be imposed on the Product by foreign and local authorities .

6. LC DIAGNOSTIC hereb v covenants to deliver in favor of PHILHEALTH the Procurement of

Periodic Health Examination for PRO IVA Employees for CY 2021 and the 'erviccs rclntcd
herem, in accordance with the technical specifications as stated in the "-\nnexes of this contract.

'· Contract duration is siC~:ty (60) six ty calendar days and will start from the 1ssuan ce of Notice to
Proceed by PHILHEALTH.

8. PHILHEALTH shall remit LC DIAGNOSTIC the payment for expenses incurred by its
employees ba~ ed on the t\CTUAL SERVICES rendered to PHILHEALTH's emplovees within
thirry (30) '..vorking days from receipt of re spective billing statements, pro1-ided that the
corresponding bookbind of the alph<tbeticallv ordered summarized list ofi the examination re,tdts and
other required documents, if any, are attached in accordance w1th the existjng accounting ~tnd
auditing rules and regulations.
