AFFIDAVIT OF EXPLANATION

I, GLADY MAE ILAGAN CATAQUIAN, Filipino citizen, of legal

age, single, with current residence and postal address at 4-10 Sunflower
Drive, Town and Country Homes, San Fernando City, Pampanga, after
having been duly sworn in accordance with, do hereby depose and state that:

1. In September 25, 2018, I migrated to Kawasaki, Japan in order to

finish my study of the in Japanese Language;

2. Thereafter, in year March 23, 2019, I return back to the

Philippines because my biological father who was already a
senior citizen have died from an unfortunate car accident;

3. By reason thereof, it is necessary for me to use my travel

documents such as Travel Document T.D. No. 17220000144 and
Philippine Passport No. P2631960 issued on April 7, 2017 at
DFA, Angeles City.

4. Further, the said travel documents were never used as a security

to any obligation or transaction;

5. I am executing this Affidavit to inform the Department of Foreign

Affairs (DFA), Philippines regarding any transaction/s and history
of the use of said travel documents as a requirement for my
application of a new Philippine passport;

6. Finally, I am executing this affidavit to attest to the truth of the

foregoing statements and for all legal intents and purposes it may
serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 4th day
