AFFIDAVIT OF GUARDIANSHIP

I, ROBERT DONALD BAXTER, Canadian citizen, of legal age, with current residence
and postal address at VDP Residences Apt. 17 Villasol Subd., Brgy., Anunas, Angeles City,
Pampanga, after having been duly sworn in accordance with, do hereby depose and state that:

1. I am the sister of ______, who is suffering from a Cerebral Palsy and considered as a person
with disability (PWD) who is currently applying for a Philippine Visa Extension. I have
hereunto attached our Certificates of Live Birth;
2. Also, on _______ I Contracted Marriage and from thereon I used the surname of my
husband. Thus, currently I am using the name ___________.
3. I am the guardian of the above-named PWD;
4. The said PWD is under my care and custody;
5. I am competent to apply and facilitate the process in behalf of the said PWD her Application
for Visa Extension;
6. I have not been diagnosed nor committed to a hospital or institution for imbecile, insane, a
vagrant or vicious person. I am not a habitual drunkard or a habitual criminal, and have not
abandoned, neglected or refused to support said PWD or caused her to commit offenses
against the law; and
7. I am executing this affidavit to confirm and affirm the foregoing facts and to prove that I am
the legal guardian of the above-named PWD;
8. Finally, this affidavit is executed for the purpose of attesting to the truth of the foregoing
facts and for whatever legal purposes it may serve.
