JOINT-AFFIDAVIT TO CLAIM SENIOR HIGH SCHOOL VOUCHER

UNDER THE SENIOR HIGH SCHOOL VOUCHER PROGRAM (SHS VP)
OF THE DEPARTMENT OF EDUCATION (DEPED)

WE, JOHN CHRISTIAN RECTO SALVADOR and MA CARLOTA MENDOZA

SALVADOR, both Filipino citizens, of legal ages, married to each other, with current residence
and postal address at 1-5 Ramon Dizon St. Diamond Subdivision Balibago, Angeles Pampanga,
after having been duly sworn in accordance with law, do hereby depose and state THAT:

1. We are the biological parents of our legitimate child ZEA MARIE SALVADOR
who is under our parental authority and custody pursuant to the Family Code of the
Philippines as amended;

2. Our legitimate child ZEA MARIE SALVADOR is currently enrolled at the

University of Sto. Tomas situated at España Boulevard, Sampaloc, Manila;

3. Our legitimate child ZEA MARIE SALVADOR is an incoming Senior Highschool

(Grade 11) for the incoming school year 2022-2023;

4. Having said this, we are availing and claiming for the Senior High School Voucher
under the Senior High School Voucher Program of the Department of Education
(DEPED) in favor of our said child;

5. We are executing this affidavit to attest to the truth and veracity of the foregoing
statements and for all legal intents and purposes it may serve.
