immunization records for yourself
or your child under 18 years.

Submitting out-of-province or out-

of-country immunization records
helps to make sure your health
record is fully up-to-date. It also
provides an accurate record of who
has been immunized.

How to get a record of your

immunizations

If you are age 14 years and older,

you can print a copy of your
immunization records from
MyHealth Records. If your child is
under age 14 years or does not
have a MyHealth Records account,
you can contact your local public
health o!ice, pharmacist, or family
doctor for a copy of their
immunizations or ask at their next
immunization appointment.
