We want to make sure all parties remain safe with minimal risk of cross contamination during viewings. In
order to ensure this is possible, we need your help.

Are you or have you/anyone who will attend the viewing with you:

• currently showing or suffering from Covid-19 symptoms? Y/N

• have shown or suffered from Covid-19 symptoms in the last 14 days? Y/N
• currently self-isolating? Y/N
• self-isolated in the last 14 days? Y/N
• currently shielding Y/N
• been in touch with anyone with Covid-19 symptoms during the last 14 days?

Please note: If the answers to all of the above questions is ‘no’, simply reply no to all of the above.
However, if the answer is ‘yes’ to any of the above, please specify which one and we will follow up
accordingly to discuss arrangements.

Do you and anyone who will attend the viewing with you:

• have personal protective equipment (face covering, gloves and hand sanitiser) that you will bring
with you to the viewing appointment? Y/N
(please note that unless you say yes the viewing cannot go ahead)
• meet the Government’s definition of an extremely vulnerable person?* Y/N

Social distancing guidelines must be observed at all times during the appointment.

Under the present conditions we are having to collect more of your personal data than usual but it will
be used only for very strict safety-related purposes and we are adopting measures to keep it secure.
