Vulvar cancer is an uncommon gynaecological malignancy with an annual incidence of 2 per 1 lakh
woman. Squamous cell carcinoma is the most common histological type constituting 90% of invasive
vulvar cancers followed by malignant melanoma which constitutes 5-6%. Spindle cell melanoma is
one of the most aggressive neoplasm which arises from skin melanocytes. The occurrence of this
tumour in the female genital tract is quite uncommon as it usually involves elderly women. It is
important to recognise and give comprehensive diagnosis as it has poor prognosis and requires a
different therapeutic approach.

We hereby present an unusual case of a young 32 year old woman, which is not the usual age group
for its presentation, diagnosed with spindle cell melanoma of vulva so as to create awareness among
clinicians about this perplexing entity. She presented with complaints of a trivial occasionally painful
4x2cm swelling in labia for a week. She then underwent excision of the same and came back with
histopathological and IHC reports showing features suggestive of spindle cell melanoma of vulva
which was managed by wide local excision following a pudendal flap reconstruction.
