Re: Mrs. Delma Eggleston, DOB: 18 March 1972, Married

Dear Dr. Kretser,

This is to refer Mrs. Kretser, who was diagnosed with Lung Cancer Stage 2A. She requires your expertise
to evaluate and manage her condition.

During her clinic visit, Mrs. Eggleston complained of shortness of breathing associated with intense
cough that a chest pain can be felt and radiates to her shoulder and back. A change in voice and a harsh
sound could be heard each breathing, sputum appearance and volume change too. Thus, chest x-ray and
CT scan has been made and results showed to be positive.

For reference, Mrs. Eggleston has been diagnosed with Type 2 DM and has been taking Metformin and
has Ramipril for her hypertension. Mrs. Eggleston does not drink and smoke and her husband is also a
non-smoker too. However, Mrs. Eggleston sometimes takes betel quid.

Given the patient’s situation, it would be appreciated if you could assess and further manage Mrs.
Eggleston’s condition. Please also note that she has a tumor and is about 5.5 cm in size.
