‘COMPANY CELL PHONE/PERSONAL CELL PHONE REIMBURSEMENT POLICY

United Service Companies (“the Company”) recognizes that certain designated employees may occasionally be
required to send and receive business text messages or use their phone as part of thelr personal scheduling and
time and attendance monitoring
The Company will pay a flat amount of $5.00 per month to reimburse employees who are regularly required to
Use their personal cell phones as part of their personal scheduling and time and attendance monitoring, If the
reasonable value of an employee's required use of his or her cell phone exceeds the stipend of $5.00, the
‘employee should obtain approval from his/her supervisor or manager to submit excess charges on an expense
report. A copy of the employee's cell phone bill must be attached with the expense report.
Employees are prohibited from using cell phones for personal calls, text messages or games during work time
(except during meal and rest periods). Company policies concerning harassment, discrimination, retaliation,
trade secrets, confidential information and ethics apply to employee use of personal cell phones. Employees
are expected to protect personal devices used for work-related purposes from loss, damage or theft.
Employees are expected to follow applicable local, state and federal laws and regulations at all times, including
the use of hands-free devices while driving. Employees who are charged with traffic violations resulting from
the use of their personal cell phones while driving will be solely responsible for all liabilities that result from such
actions.
Employees who work in hazardous areas must refrain from using personal devices while at work in those areas,
as such use can potentially be a major safety hazard.

receive or provide business related information during my scheduled shift. | understand that I will
receive amount of $5.00 per month to reimburse me for business use. | understand that if this amount
does not fully reimburse me for the reasonable value of the use of my personal cell phone, | should
request approval for reimbursement of the excess charges.
