It is extremely concerning that the individual's safety culture has


deteriorated to the point where someone does not recognise the risks associated with the mooring rope
under tension. Please meet with the HoD to develop an action plan to prevent such NM and real
incidents from occurring in the future. If necessary, follow the just culture tree and meet with the
individual.

I would like to remind you of a fire incident on a sister ship that was caused by oily rags left in the
incinerator ash bin. During the investigation, a significant amount of risk normalisation was observed
despite the incident's severity. Please be aware that we recently ran a campaign to improve the culture
of housekeeping onboard. Please review the materials and distribute them to all  as a refreshment.
Across the fleet, we observe housekeeping as an area of concern and one on which we must place
greater emphasis in order to enhance the overall safety culture onboard. Please discuss with everyone
onboard and emphasise how minor hiccups can lead to catastrophic unintended outcomes. Additionally,
please increase engine and deck rounds to monitor compliance and trends.

NM noted that apart from the discussion during the next safety meeting, please discuss the importance
of intervention amongst crew members and prompt actions. Even this looks slight deviation from the
procedure; it reflects pretty much safety culture onboard, a tendency to do shortcuts and deviate from
the standard agreed working practices. Also, these types of NM raise concerns on TBT quality – please
discuss with all head of departments.    recommend that senior officers increase rounds and demonstrate
visible leadership by leading by example. Please use just culture tree when the actions were deliberate.
