1. Pay the Testing Fee (300.00) at the University Cashier-DHVSU Bacolor Main Campus before your scheduled University Admission Test. Please secure a copy of Official
Receipt to be presented to the proctor on your examination day.

2. Take note of your student application number.

3. Come 15 minutes before the scheduled time. Failure to appear on the given schedule would mean the cancellation of your permit. Rescheduling is subject to the availability
of slots.

4. Bring the following:

a. One Valid ID
b. Printed University Admission Test (UAT) Permit
c. Official Receipt/Proof of Payment of Testing Fee
d. Pencil, Ball pen, and eraser
e. 1 pc 2x2/passport size picture

5. USING CELLULAR PHONES AND OTHER ELECTRONIC GADGETS IS STRICTLY PROHIBITED WHILE INSIDE THE TESTING ROOM.

6. For the attire: White top with collar and black pants. Slippers, sandals, and the like are not allowed for the footwear.
