to in accordance with law, do hereby depose and state THAT:

1. I am the proprietor of NESSA GARDEN, duly established based on the existing laws,
rules, and regulations of the Republic of the Philippines, with business address at Gaisano
Grand Mall, Purok Chico, Brgy. Sto. Niño, Panabo City;

2. The said business endeavor has no other employee except me, who is personally handling
all its operation;
