8. Are the crane accessories capacity equal / more than load weight? YES NO
9. Is the load weight not exceeding 75% of the crane capacity as per the set- YES NO
up (If exceeding 75%, critical lift plan available)?
10. Are the out riggers placed, away from shallow or dangerous underground YES NO
services?
11. Is the boom height restrictions communicated to the operator for the YES NO
overhead obstructions and is implemented?

Note: A specific lifting permit is valid for a particular lift in a precise site location and won’t be valid for similar lifting in
different site locations by using a single permit; separate permit shall be obtained for diverse locations, loads and lifting
equipment. Nonetheless typical shaped loads, same or below the permitted load with the same positioning of the crane
can be allowed under a single lifting permit.
