3. That the documents submitted are certified photocopies/copies of the original, or as
specific part thereof, as the case may be.
4. That the requested documents pertaining to Procurement of Security Services for Sucat
Thermal Power Plant was made sometime 2016 and the payment of security services was
made sometime 2017;
5. That the foregoing document certified is submitted to the Office of The Ombudsman in
relation to Subpoena Duces Tecum (FF-C-18-0037) dated November 10, 2022 signed by
Ferdinand Q, San Joacquin, Director IV, GIB-D, Office of The Ombudsman, received by the
Office of The Supervising Auditor Commission on Audit (COA) PSALM Corporation on
November 16, 2022;
6. That the submission of such document is in compliance of legal order from The Office of
The Ombudsman.
