Since one of the objectives of the law is to facilitate the process, the forms were devised in such a way that even without a lawyer or legal assistance, the pelifioner will be able to undergo the process. In addition, the forms are provided to ensure that there will be no gaps for the required iniormation relating to RA 9048. Finally, the forms serve as a means fo @void confusion in the application of the law, and in the review of petitions. in view of the above, the following rules to govern the use of the forms are hereby reiterated. |. All petilions for Correction of Clerical Error/Change of First Name/Appeal/Motion for Reconsideration shall use RA 9048 Form: as appearing in the appendix no. 1 of the Manual of Instructions which are as follows: 2.
