2) I am a FIRST COUSIN of MR. RICARDO DALISAY JR. who was born on

3) The abovementioned person will be traveling to SINGAPORE upon my invitation to visit

me and for tourism on 28 FEBRUARY 2019 and will stay for a period of ONE (1)
MONTH.

4) I am sponsoring their trip including the airfare, accommodations, tours, and meals; and
that I am financially capable to support their stay.

5) I guarantee that for the duration of their stay, they will not violate any existing laws of
SINGAPORE;
