AFFIDAVIT OF KINSHIP

I, MANUEL L. LAURENTE, JR., Filipino, of legal age, married, and a resident

of Phase 8, Block 9, Lot 49, Deca Homes, Mintal, Brgy. Tacunan, Davao City,
Philippines, after having been duly sworn to in accordance with the law, do hereby
depose and state that:

1. My parents were Manuel Laurente Sr. and Erlinda Lagura Laurente who
respectively died on December 13, 2020 and August 03, 2002;

2. I have three (3) other siblings namely: Michael Laurente, Maria Cristina
Laurente, and Mark Laurente;

3. Being the nearest kin of my brother Mark Laurente who has been mentally ill
and had been admitted in Southern Philippines Medical Center (SPMC) -
Institute of Psychiatry and Behavioral Medicine (IPBM), Davao City,
Philippines, I hereby execute this Affidavit to attest to the truth of the
foregoing and for the following purposes:

 To request a copy of my brother Mark Laurente’s Birth Certificate and

all civil registry documents in that regard with Philippine Statistics
Authority (PSA);
 To request for Medical Certificate of my brother Mark Laurente; and
 For such other legal purposes this Affidavit may serve.
