MEDICAL REPORT
RE:MISS ORJI, INNOCENTIA OGECHI

The above named is a patient of the Casualty Unit of the hospital. She
was first seen on 24th April, 2 023 on account of fever, internal
bleeding, Clinical findings and laboratory investigation shows a
diagnosis of several headache an d internal bleeding on the head was
managed conservatively with all modalities of treatment he was placed
on recovery medication.
We wish to advice that he should not be subjected to strenuous
activities/exercise such as having to travel every week for treatment.

please accord him the necessary assistance she needs to enable her
redeploy to Imo State near his relatives and their family doctor for
adequate and total monitoring of her conditions while serving her father
land.
