A collaborative process is used to develop policies and / or


procedures that address the accuracy of patient identification.
 Use at least two (2) ways to identify a patient:

i) giving medications
ii) giving blood and blood products
iii) taking blood samples
iv) taking other samples for clinical testing
v) providing treatment or procedure

 The patient’s Room / Bed Number cannot be used as an identifier

A collaborative process is used to develop policies and/or procedures that


address the accuracy of verbal and telephone communications

 Person receiving the following:

i) Verbal order
ii) Telephone order
iii) Reporting of critical test results

 Must use a verification “read back” of complete order or test result .


 The order or test result is confirmed by the individual who gave the order
or test result.

 A collaborative process is used to develop policies and/or


procedures that improve the safety of high-alert medications
including concentrated electrolytes

 High Alert Medications


i) Involved in a high percentage of errors and/or sentinel events

ii) Carry a higher risk for adverse outcomes

iii) Look-alike sound-alike medications(LASA)

 Concentrated electrolytes are not present in patient care units


unless clinically necessary and actions are taken to prevent
inadvertent administration in those areas where permitted by policy

 Remove concentrated electrolytes from patient care units,


including, but not limited to, the following:

 Potassium Chloride > 2 mEq/ml


 Potassium Phosphate
 Sodium Chloride > 0.9%
 Magnesium Sulfate > 50% concentrate

 Develop checklist to verify correct documents and functioning


equipment preoperatively

 Mark the precise site and involve patient in doing this

before surgical procedure

The purpose of the preoperative verification process is to:

i) Verify the correct site, procedure, and patient;

ii) Ensure that all relevant documents, images, and studies are
available, properly labelled, and displayed; and

iii) Verify any required special equipment and/or implants are


present

A collaborative process is used to develop policies and/or


procedures that address reducing the risk of health care–
associated infections
 The organization has adopted or adapted currently published
and generally accepted hand hygiene guidelines
 The organization implements an effective hand hygiene program
• Reduce the risk of patient harm

The organization develops an approach to reduce the


risk of patient harm resulting from falls.

Falls account for a significant portion of injuries in


hospitalized patients.

 Fall history, medications, gait and balance screening,


walking aids used by the patient and inappropriate use
of physical restraints

Assess and periodically Reassess each patient’s risk for


falling, including the potential risk associated with the
patient’s medication regime, and take action to decrease
or eliminate any identified risks.
