Subject: Delivery Personnel's Entry Requirements Series 2-06082020

As part of our control measures in this period of pandemic, please be guided by the following.

1. All delivery personnel shall be wearing face masks when they enter all sites of MDC and its
subsidiaries.
2. All delivery personnel shall be logged and their temperatures will be checked at the entrance gates by
MDC MAPD Security personnel.
3. All delivery personnel shall ONLY be allowed entry in DELIVERY ZONES.
4. All delivery personnel shall be wearing at all times face masks and remain inside their vehicles during
receiving when practical.
5. All delivery personnel are required the following:
a. Accomplish the MDC Health Declaration Form (HDF)
b. Checked, approved, and signed HDF by CMHS OHP or designated First Aider.
c. Wear all required Level 3 Biohazard PPEs* once they alight from their vehicle as follows:

i. Full face shield.

ii. Surgical masks

iii. Disposable Isolation gowns

iv. Disposable latex gloves

v. Shower cap

*All Level 3 Biohazard PPEs should be supplied by the employer, kept clean and sanitized.

*Disposable Level 3 Biohazard PPEs should be of single use and replaced every re-entry.

d. Wear Standard Construction PPEs as required once they alight from their vehicle.
