AFFIDAVIT OF ADMISSION OF PATERNITY / RECOGNITION

AND TO ALLOW THE USE OF SURNAME

I, ROEL E. PAGATPAT, Filipino, single, legal age and a resident

CONSOLACION, CEBU after having been sworn to in accordance with law, do
hereby declare that;

1. I am the biological father of ROEL ONGGO.

2. The mother of ROEL ONGGO is REBECCA M. ONGGO.
3. He was born on May 7, 2017 at Valencia, Bukidnon. Her Birth Certificate is
duly registered with Registry No. 2017-1322
4. REBECCA M. ONGGO (mother) and I do not have legal impediment to marry
each other when ROEL ONGGO (child) was conceived.
5. ROEL ONGGO (child) has been living with me since she was a baby. She is
now Five (5) years old.
6. That being the father of ROEL ONGGO (child) I hereby acknowledge and
recognize him/her as my child and legal heir;
7. That I am allowing my child to use my surname “PAGATPAT” to be his/her
surname in his/her Certificate of Live Birth, school records and all public
documents.
8. That I execute this affidavit to attest to the truth of the above facts in
compliance with the provisions of Republic Act No. 9255.
9. I hereby certify that the statements made herein are true and correct to the
best of my knowledge and belief;
10. This Affidavit is explained to me and I understand its content.
