minimization of risks associated with
programs and core mandates related to

UNDERSTANDING THE ORGANIZATION AND ITS
CONTEXT:

Southern Philippines Medical Center (SPMC) is a


government hospital in the island of Mindanao, also
considered to be the largest in the Philippines with its 1,500
bed capacity. Our vision is to become a world class service
oriented apex hospital leading in the provision of
healthcare for all Filipinos.
The provision of health care is not without risk. Every
year, millions of patients suffer injuries or die because
of unsafe and poor-quality health care. Many medical
practices and risks associated with health care are
emerging as major challenges for patient safety and
contribute significantly to the burden of harm due to
unsafe care, (WHO, 2019).
CORE MANDATE:

Patient Safety is a fundamental element of health care


and is regarded as an essential component for
improving health outcomes. The provision of safe and
quality health service that is responsive to the needs of
the people is a vital pillar in helping achieve universal
health care (DOH, 2022).
BASIS FOR RISK MANAGEMENT:

DOH AO No. 2020-0007 “National Policy on Patient


Safety in Health Facilities”

Identifies Risk Management as a key element of the


program wherein clinical and administrative activities
are undertaken to identify, evaluate, and reduce the
risk of injury to patients, staff, and visitors and the risk
of loss to the organization itself.
BASIS FOR RISK MANAGEMENT:

IS0 -9001 - Quality Management Systems

This International Standard employs the process


approach, which incorporates the Plan-Do-Check-Act
(PDCA) cycle and risk-based thinking. As well as
address risks and opportunities associated with its
context and objectives.

Hospital Memorandum No. 227 series of 2017 also


known as “The Creation of Patient Safety And
Pharmacovigilance Unit”

• 1 Clinical Pharmacist
• 1 Administrative Staff
OVERSIGHT:

PSPVU is given authority and responsibility to perform risk


management activities to mitigate risks due to adverse events
and learn to prevent future recurrences.

“Patient Safety and Pharmacovigilance Unit Functions.” )
