the company with a copy of his personal physician’s medical
assessment dated September 27, 2022. A copy of the medical
assessment is hereto attached as Annex “A”.

Likewise, Mr. _____ would like to manifest his willingness to

submit to a third doctor if needed under the circumstances and under
parameters agreed to by the parties. As such, a copy of the
seafarer’s proposed parameters for third doctor referral is hereto
attached as Annex “B”.
