CIRCULAR

I am directed to inform all staff to take note of the following instructions

1.Each staff is expected to download the NHF2 Form from FMBN website
www.fmbn.gov.ng and fill with the Employer number 2000002640

2. Any staff with previous NHF employee number should write the number on the
topmost corner of the download form (NHF2)

3. Each staff should attach Two (2) passport photographs and a photocopy of any of
the following documents

I. National ID Card
II. International Passport
III. Driver’s License

Please note that the ID document should be photocopied at the bank of the NHF2
Form

Each staff is expected to complete and submit the on or before Friday 4th January
2022 to the Human Capital Management and Development Department
