To ensure that you are following Dr.

Nash's instructions properly, we require the


following:

During Reduce Phase four (4) weekly food logs must be sent via the
Voxer App or email at the end of each week of the Reduce Phase
Program
The office staff works remotely and will not be able to remind you to send
your food logs. This is 100% the responsibility of the client
During Retrain Phase a food diary application (via smartphone, tablet,
laptop, or desktop) is highly recommended to track calorie level and
macronutrient percentages to ensure maximum metabolic correction
It is the client's responsibility to maintain the Retrain Phase food diary for
the duration of the Retrain Phase

I have read the above and I understand that I am 100% responsible for
recording my food intake for the duration of my TOTAL Transformation
Program.
