INSTRUCTIONS

1. Bidders shall submit the certified true copy of the following documentary
requirements upon submission of quotation:

a. Valid and current Mayor’s/Business permit issued by the City or Municipality


where the principal place of business of the bidder is located, or the equivalent
document for the Exclusive Economic Zones or Areas;
b. Document showing the bidder’s PhilGEPS Registration Number;
c. Duly accomplished Technical Specifications (Annex “A”);
d. Duly accomplished Schedule of Requirement (Annex “B”); and
e. Duly accomplished Financial Bid Form (Annex “C”).

*Note: A valid Certificate of Platinum Membership with PhilGEPS may be submitted


in lieu of items a and b

2. The quotations must be enclosed in a sealed envelope and received on or before


10:00 AM, 3 August 2023 by hand-delivery or courier at the following address:

Quotations received after the above-mentioned deadline will not be accepted.

TERMS AND CONDITIONS

1. Bidders shall provide correct and accurate information required in this form.

2. Any interlineations, erasures, or overwriting shall be valid only if they are signed
or initialed by you or any of your duly authorized representative/s

3. The price quotation should be valid for one hundred twenty (120) calendar days
from the deadline of the submission indicated above.

4. Price quotation/s, to be denominated in Philippine peso, shall include all taxes,


duties, and/or levies payable.

5. Quotations exceeding the Approved Budget for the Contract shall be rejected.

6. In case of two or more bidders are determined to have submitted the Lowest
Calculated Quotation/Lowest Calculated and Responsive Quotation, the PNR BAC
shall adopt and employ “draw lots” as the tie-breaking method to finally determine
the single winning provider in accordance with GPPB Circular 06-2005.
