Antibiotic solution lock therapy

Vancomycin 2.5mg/ml + Heparin 2500 units/ml

Instructions:
Dilute 500mg of Vancomycin with 10ml (to make 50mg/ml)
Remove 1 ml and further dilute this with 9ml on NSS (to make 5mg/ml solution) – label
as Solution A
Draw up 1.5cc of 5000units/ml Heparin into a syringe and mix with 1.5ccof Solution A
to make 3cc of final solution

Administration:
Prior to installation of antibiotic lock, withdraw contents from catheter lumen
Flush catheter with normal saline
Instill antibiotic lock solution to fill the catheter lumen ~2cc
Label the catheter “Do not use” – antibiotic lock
Aspirated lock solution from catheter lumen and flush with saline before using line
