JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ____________________ and ____________________, both of legal ages, Filipinos,

and residents of _______________________________________________________________,
and ___________________________________________________, after having been duly
sworn to in accordance with law, hereby depose and state:

1. That we personally know the late Ernand P. Castillo;

2. That there is an inconsistency in the maiden name of Ernand P. Castillo’s mother in the
documents issued by the Philippine Statistics Authority (PSA);

3. That the issued birth certificate and advisory on marriages to Ernand P. Castillo indicated
“Lilia H. Padilla” as his mother’s maiden nameon the other hand, in Ernand P. Castillo’s
birth certificate and marriage certificate indicated “Lily H. Padilla” as his mother’s
maiden name;

4. That the aforementioned documents pertain to the one and same person;

5. That Ernand P. Castillo was married to Luzmin A. Castillo;

6. That they have a daughter named, Chola Eunice A. Castillo, who was born on January 11,
1993 in the City of Mandaluyong, Philippines;

7. That on the marriage certificate of the spouses and advisory on marriages, it indicates that
their marriage date is on April 5, 1991, and on the birth certificate of Chola Eunice A.
Castillo, the date of marriage of spouses Ernand P. Castillo and Luzmin A. Castillo is on
September 11, 1991. Hence, there is a discrepancy;

8. That we are executing this Affidavit to attest to the discrepancies in the maiden name of
the mother of Ernand P. Castillo and the marriage date of the parents on the birth
certificate of Chola Eunice A. Castillo;

9. That we are not related by affinity or consanguinity to the above mentioned persons;

10. That we are executing this joint affidavit to attest to the truth and veracity of the
foregoing set of facts without any promise or reward or any other monetary consideration
but solely for whatever legal purpose it may serve.

FURTHER AFFIANTS SAYETH NONE.
