1.0 Purpose

The purpose of this document is to ensure the documentation of the activities will give engineers and
managers the capability to learn about service practices. With this knowledge DFSP field services can
make decisions to increase efficiency and customer satisfaction. When field tickets, mud, SCE and Waste
Management reports are filled out correctly and accurately, they provide the first link in the chain of the
account receivable process.

2.0 Scope

This document applies to all operations in DFSP.

3.0 Guidelines

Reports are filled out and the following should be considered.

 If in doubt on how to fill any specific report ask your immediate supervisor.
 Daily calls and debrief meetings are good opportunities to clarify report content.
 Pricing should be applied according to service proposal, written agreement or
contracts with customer, or DFSP price list.

4.0 Performance Criteria

4.1 Reports must be accurate, timely and fully completed.

4.2 The following reports must be completed:

 Initial Inventory (chemical and spare parts)

 Technical Service Order
 Daily Mud, SCE / Waste Management Report and Inventory
 Material, Spare Parts Requirement Order
 Correction prevention improvement
 Safety Meeting Reports
 End of Well Inventory
 Local Regulatory Requirement
 Any further supervisor designated requirements
 End of Well Recap
 Any Associated Accident / Incident Reports
 Lessons Learned Log
