AFFIDAVIT OF TWO DISINTERESTED PERSONS

Municipality of Panglao, Province of Bohol, Philippines and______________________,
likewise Filipino, of legal age, married, and a resident of Brgy. Tangnan, Municipality of
Panglao, Province of Bohol, Philippines, after having been duly sworn to in accordance with
law do hereby depose and say:

1. That we personally know a certain MANUEL ULGASAN PASICARAN;

2. That in his Certificate of Marriage, Birth Certificate and Driver’s License, his name
was entered as MANUEL ULGASAN PASICARAN;
3. That the names MANUEL ULGASAN PASECARAN and MANUEL ULGASAN PASICARAN
refers to one and the same person;
4. That we are executing this affidavit in order to attest the foregoing facts and for
whatever legal purpose this may serve:
