isk Assessment and Hot Work Permission
A recently submitted Level 2 SFSAF 28 Risk Assessment with SFSAF 25 Hot Work Proposal
highlighted two issues:
1) Level 2 SFSAF 28 Risk Assessment
‘The formula used in the risk assessment section which enables the risk level to be calculated
had been removed. The risk level had then been typed in though did not correspond with the

If after selecting the hazard consequence and hazard likelihood the risk level is not
sutomatically calculated then the form is corrupt.
Action: A protected version of this form has been attached to this email. Plea
remove any existing blank Level 2 SFSAF 28 forms from your network and replace with
this one. The controlled version of the new SFSAF 28 will be sent out on the next
update to the Stationary Disc.2) The SESAF 25 Hot Work Proposal ~ Office Approval Process: Who Can Approve?
When a Superintendent receives a Hot Work Proposal from a vessel they are required to co-
ordinate the Hot Work review and ensure authorisation is granted as per the following table
which has been taken from M009 3.4.12 and M101 4.3.4.9:

“Note: The approval levels noted are the minimum level required. If this person is not available, the
request for approval should be escalated to a higher position.
The changes above were published in Fleet Circular No. 89.
‘The SFSAF 25 Hot Work Proposal form is currently being revised to reflect the authorisation
levels for each Class and type of vessel. While this is underway, please ensure the above
procedures are followed.
