1. PURPOSE
The purpose of this standard is to provide minimum requirements for energy isolation application in protecting
employees and equipment from the hazards of energy sources and hazardous and/or toxic materials.
2. SCOPE
2.1. This standard address minimum requirements for the application of hazardous energy sources and
hazardous and/or toxic materials isolation.
2.2. This standard applies to:
a. BP AKR Business Units, Partnership and Contractor Companies who work at BP AKR work area.
b. Employees and other individuals (including temporary employees and contractor personnel),
company’s visitors, or any other person(s) who work and/or are present in the workplace.

3.4. Tag-out Device – a prominent warning device, such as a tag and a means of attachment, which can be
securely fastened to an energy isolating device to indicate that the energy isolating device and the
equipment being controlled may not be operated until the tag-out device is removed.
3.5. LOTO Tag – Tag used for LOTO (Lock Out Tag Out) with the employee’s name, ID Number, and
employee’s Business Unit or Company’s name.
3.6. Energy Isolating Device – a mechanical device that physically prevents the transmission or release of
energy, including but not limited to the following: circuit breaker, a disconnect switch, a line valve, and
any similar device used to block or isolate energy.
3.7. Designated Person for energy isolation/LOTO Group Application – Crew Supervisor performing
energy isolation/LOTO. In an absence of the Crew Supervisor or when there is more than one tasks
requiring energy isolation to perform, the Crew Lead-hand or an employee competent in energy
isolation/LOTO can be appointed to be the Designated Person.
4. STANDARDS OF PERFORMANCE
GENERAL REQUIREMENTS
4.1. Energy isolation shall be required whenever service, maintenance, modification, or installation activities
are performed on de-energized equipment in which unexpected energizing, starting-up of the equipment,
and/or the release of stored energy may occur with the potential to cause an incident.
4.2. Energy isolation shall be implemented by applying both a mechanical lock and tag in combination. Tag-
out application without a mechanical lock is not considered an effective lockout application, and such a
practice is unacceptable.
4.3. A thorough risk assessment to identify energy isolation/LOTO requirements, all sources of
hazardous energy and availability of energy isolating devices shall be carried out (see Attachment
10.2 Energy Isolation/LOTO Survey Form).
a. In a case where an energy isolating device is not possible to be locked out, it must be equipped with
another effective device/guard to which energy isolation can be effectively applied.
b. Specific SOP shall be developed to regulate energy isolation application in a facility or equipment with
multiple energy sources or if there is any specific work situation where normal energy isolation
procedure cannot be implemented. The SOP must afford a similar level of protection as provided by
energy isolation.
4.4. Energy isolation shall only be applied and removed by the person it is intended to protect.
4.5. Only individuals who have received appropriate energy isolation training shall be allowed to
implement energy isolation application.
a. Energy Isolation Refresher Training for Energy Isolation/LOTO shall be completed by each
individual performing energy isolation every two (2) years.
4.6. Energy isolation devices shall be singularly identified and shall be the only device(s) used for safeguarding
potential energy sources.
a. Energy isolation devices shall not be used for other purposes.

b. Energy isolation/LOTO tag shall be made of a water-proof material that prominently display the
personal ID Number, Name of the Owner, the Division/Department he/she works with, and the expiry
date of the tag (at maximum for 2 years, or when Energy Isolation/LOTO refresher training is required).
4.7. Energy Isolation/LOTO padlocks shall be coloured red to distinguish it from locks used for other
purposes.

e. Perform your assigned task(s) on the de-energized machinery or equipment.
f. When the task or tasks have been completed, each individual shall remove his/her own energy
isolation/LOTO.
4.11. Main steps for Group Energy/LOTO Application will be as follow:
a. Each work crew shall nominate a Designated Person for energy isolation/LOTO Group
Application.
b. Disconnect the energy flow at the Energy Isolating Device(s).
c. The Designated Persons shall apply energy isolation/LOTO to the Energy Isolating
Device(s).
d. The Designated Person shall carry out a POSITIVE TEST at primary switches/buttons/valves to
ensure no stored energy remains on the de-energized equipment.
e. The Designated Person shall complete the Group energy isolation/LOTO Information Sheets (See
Attachment 10.1 – Group LOTO Information Sheet) and attach the completed sheet onto the
designated energy isolation/LOTO Box.
Notes:
The sheet contains information about the types and number of Energy Isolating Devices locked
out, and the signatures of respective Designated Person.
Energy Isolation/LOTO Box shall be made of durable material and is intended to keep the
energy isolation/LOTO keys owned by the Designated Persons. The Energy Isolation/LOTO box
contains holes along the extended lid cover of the box where employees can apply their energy
isolation/LOTO.
f. The Designated Person shall store his/her energy isolation/LOTO key(s) inside the energy
isolation/LOTO Box.
Note:
The number of energy isolation/LOTO keys shall match the number of locks used on the Energy
Isolating Devices as indicated on the completed Group Energy Isolation/LOTO Information
Sheets.
g. All employees shall then apply their respective energy isolation/LOTO by locking through the
perforated holes along the extended lid cover of the box.
Note:
Each employee shall read the Group Energy Isolation/LOTO Information Sheets and
count the number of keys stored inside the box, prior to applying his/her LOTO.
Do not apply your energy isolation/LOTO when in doubt but immediately consult your
respective Designated Person or your Immediate Supervisor, instead.
h. Perform your assigned task(s) on the de-energized machinery or equipment.
i. When the tasks have been completed, each individual shall remove his/her own energy
isolation/LOTO from the box.
j. The Designated Person(s) shall then pick up his/their energy isolation/LOTO keys from the box and
remove their energy isolation/LOTO from Energy Isolating Device(s).

LOTO DURING EQUIPMENT COMISSIONING/LIVE TESTING
4.12. In a case of an equipment, machinery or facility requires to be commissioned or tested in a live mode
(partly or fully energized), where normal energy isolation/LOTO application is impractical to apply,
the following steps shall take place:
a. Project Coordinator shall carry out a thorough assessment to identify which part of task(s) that
normal energy isolation/LOTO application is considered impractical.
b. Develop Job Safety Analysis (JSA) to address not being able to lock out.
c. Communicate the JSA to all relevant employees/parties to gain thorough understanding of what
will be done in lieu of not locking out.
d. Project Coordinator shall assign a Competent Person to lead and supervise the
commissioning process.

EMPLOYEE TRAINING
4.21. All employees performing energy isolation/LOTO, including supervision and Management personnel,
inspectors and other people who will be taking part in energy isolation/LOTO application shall receive
energy isolation training. The following types of energy isolation training and awareness program will
be applied site-wide:
a. Upon initial job assignment – General Energy Isolation Program Training and
Supplemental Procedures that are applicable to the employee and his/her work area.
b. Upon transfer to new work area – particularly for obtaining appropriate understanding of
supplemental procedures.
c. Whenever there are changes in general energy isolation procedures or supplemental
procedures.
d. Two-yearly Refresher Training for maintaining and communicating information of general and
supplemental energy isolation procedures.
4.22. Energy Isolation training documents and records shall be maintained for reference and potential audits.
5. STANDARDS OF ACCOUNTABILITY
5.1. BP AKR President Director
a. Generate BP AKR Standard to regulate energy isolation program and review it at minimum per
three (3) years.
b. Provide resources for the effective implementation of energy isolation program.
c. Ensure the program is implemented throughout BP AKR work area.

5.3. Head of BP AKR Health Safety Security and Environment Department
a. Provide HSE advice for energy isolation/LOTO program implementation.
b. Ensure energy isolation training and awareness modules are in compliance with GOI and BP AKR
Standards.
c. Develop energy isolation Training modules and organize the training, awareness and
refresher program for employees.
d. Maintain auditable documents and records of energy isolation training.
5.4. Employees
a. Understand BP AKR energy isolation/LOTO standards and procedures and how they impact their
work.
b. Attend all required energy isolation training, awareness program and two-yearly energy
isolation refresher program.
c. Ensure appropriate energy isolation/LOTO of hazardous energy is in place before work is
performed.
d. Correct and/or report any energy isolation/LOTO discrepancies observe to immediate
supervisor or relevant parties.
