AFFIDAVIT OF ARREST

I, P/Msgt. Kenji T. Ballesteros, of legal age, bonafide member of the Philippine

National Police and presently assigned at Piddig Municipal Police Station after having
been duly sworn to in accordance with the law, do hereby depose and state:

1. That on or about 5:00 AM of august 20, 2023, our station received a report
through a phone call from John Valle y Alonzo, of legal age, a crew in
Magdalena’s Food house in Barangay Maruaya, Piddig, Ilocos Norte, that a
shooting incident transpired in front of the said food house.

2. That, acting on that information, the officer in charge directed me to

respond at the reported place to verify the veracity of the said information
and if affirmative, to conduct an ocular investigation;

3. That, upon arrival at the place of incident, I were personally informed by the
witness that the victim one Jericho Yvez y Abes was already rushed to the
hospital but declared dead on arrival by the attending physician; and that, I
were likewise informed that the suspect one Junnel Pascua y Arellano was
voluntarily surrendered himself after the incident with the weapon used;

4. That, I immediately informed our station conforming this incident; and that
on August 20, 2023 at about 8:30 AM, the suspect appeared and
surrendered himself to Piddig Municipal Police Station.

5. That about 9:00 AM, we executed this affidavit to attest the veracity of the
aforementioned facts and to support the filing of appropriate charge/s in
court against the arrested suspect;
