I, Reynaliza G. Acera, of legal age and is currently stationed at Dampias National

High School do hereby voluntarily depose and state that;
I have unintentionally misdated check bearing the following number ________,

That due to an unanticipated distractions, I mistakenly dated the checks May
_____, _____, ____ and ______ 2023 instead of June ____, ______, ____, ____ and
____ 2023.
That, during the issuances of the checks I was convinced that the month of June
is written in numeric as 5 and not 6.
That, this honest disorientation led to issuance of the misdated checks.
That I committed the error during the issuance of the check on June _____2023

That, I therefore testify that it was an honest and unintentional mistake since the
checks is covered in the sequence of checks with the number _______ to ______
which are all issued for the month of June.
