1.0 INTRODUCTION
 
In January 2020, at KP265 the pipeline was parted due to landslide occurred and
caused the operation of SSGP being stopped immediately. Among other repair works
of SSGP, at KP265 is the major repair works needs to be performed before the
pipeline can be back into operation that is targeted in July 2022. At KP265, the major
activities are civil and geotechnical works and re-routing of the parted pipeline for a
length of approximately 340 meters.

The purpose of this procedure is to set standard to which scaffolds will be erected
using sound materials, standards for their use, establish inspection parameters and
identify co-ordination requirements so as to minimize interface problems. This
procedure is to ensure that all scaffolding and working at height will be managed
safely.
 
2.0 SCOPE OF WORK

The work comprises all elements itemized as per below:

 Provide designated access for in and out to existing ROW.


 Setup temporary working platform using scaffolding for microbe via live stake &
vegetated swale drainage installation.
 To setup temporary anchor point using scaffolding tube for microbe via live stake
and vegetated swale drainage installation.
 
3.0 DEFINITIONS

CONTRACTOR : BWS ENGINEERING SDN. BHD.
CONTRACT : The legally binding document for the work.
WORK : The scope of works under this CONTRACT, to be read
in conjunction with the drawings, specifications,
codes and standards, procedure and other related
CONTRACT documents.
FACILITIES : Shall mean all the equipment, pipeline, building and
station related details in this project.
PROJECT : Shall means EPCC of KP265 Pipeline Reroute Project
(Others & Logistic Scope)

The word shall indicate a requirement.


The word should indicate a recommendation.

5.0 ROLES AND RESPONSIBILITIES

5.1 Construction Manager 

Construction Manager is responsible to plan and coordinate the works from


the beginning until completed. The construction manager also managing the
works manpower as to ensure the works is completed within time, budget,
good quality and safe practices.

5.2 Project Engineer 

Project Engineer is responsible to overall management of the works from


engineering controlling to site execution. The project engineer also acts as
assistant to construction manager in managing the works manpower as to
ensure the works is completed within time, budget, good quality and safe
practices.

5.3 Site Engineer 

Site Engineer is responsible to manage and supervise the works and


manpower as to ensure the works is completed within time, budget, as per
technical design, good quality and safe practices.

5.4 Work Leader/Supervisor  

To obtain the PTW prior any scaffolding activity. He shall ensure that
appropriate risk assessment such as job hazard analysis is carried out prior to
erection, dismantling and modification of scaffolding and appropriate safety
measures or controls are identified and implemented.

5.5 Scaffolder 

To erect, modify or dismantle all scaffolding activity and the structure itself
shall not obstruct any emergency access way or safety equipment or cause
damage to exiting amenities.

5.6 Scaffolder Inspector 

To inspect all scaffolding before they are approved to be use. He shall


complete the scaffolding status which indicates the location, inspection date,
load rating, and safe for use.

5.7 Excavator Operator 

Excavator Operator is responsible to perform safe operation for loading and


unloading activities and shifting of equipment and material. When necessary,
excavator also will be used to erect/plant scaffold into ground.
He is also responsible to carry out daily inspection of the machineries and to
ensure that the machineries is well maintained.

8.0 COMPETENCY AND TRAINING

• Contractor or other party engaged for scaffolding erection shall be made aware
of the operational activities being carried out on site and potential hazards
through HSE induction training.
• All Scaffolder shall be trained at training institution approved and accredited by
relevant authority as per local laws and regulations. In Malaysia, the training
institution shall be registered with the Department of Occupational Safety and
Health (DOSH)
• The person working at height shall attend basic WAH training and specific training
for use of Fall Protection equipment.
• Basic work at height training shall include:
o Hazards specific to the job
o Fall prevention
o Lessons learnt from incidents
o Use of harness/double lanyard
o 100% tie-off
o Suitable anchor points
o Fall factor
o Rescue plan awareness
o Field demonstration

9.0 WORK METHODOLOGY

9.1 PROCEDURE OF SCAFFOLD INSTALLATION AT KP265 EXISTING ROW

 Site assessment shall be made to evaluate site condition before any


designing and installation begin. Drawing of site layout will be referred to
design suitable scaffold platform for area of work at Existing ROW.
 CONTRACTOR will hire a Professional Engineer to design the working
platform with detail engineering calculation.
 With refer to the endorsed design by Professional Engineer, CONTRACTOR
will proceed with mob of material and installation of Scaffold Platform to
site. Refer to ATTACHMENT 3.
 CONTRACTOR will install the scaffold platform at locations as per marked
in Photo 1. Three critical locations with high steep area at the red zone
will require to install scaffold platform as per endorsed PE design. The
three locations named as Platform A, B, and C. The location is indicative
and will be depending on site requirement and assessment. More platforms
will be installed when required.
 Scaffolders level 1 are responsible to install, dismantle and maintain
scaffolding.

 At Platform A, vertical scaffold will be first planted onto the ground with
using excavator. Then, horizontal and diagonal scaffold will be fixed with
using swivel clamp. Metal deck will be installed for walking platform. For
pipe that is not accessible by excavator, scaffolder will use manual method
by hammer.
 Ladder will be then installed as per design.
 Next, rope for safe life line for worker to hook their lanyard will be installed
by tying properly at the anchor point. Ladder with hook will be installed as
per Photo 2.

 Step 5 to 8 will be repeated at Platform B and C.


 Upon completion of scaffold platform installation, Level 3 scaffolder will
inspect with refer to the endorsed PE design and label the scaffold with
green tag.
 CONTRACTOR shall ensure all workers have full PPE before use the
platform for work.
 Double front body harness complete with double lanyard shall be use at all
time and the lanyard should always hooked to the rope during work.
 Supervisor shall monitor at all time when workers using the scaffold
platform.
 Materials will be stored at control post area and will only be mob to site
during installation using safe access with designated safe inspected
vehicles.
 Before erection starts, the scaffolding material shall be inspected to ensure
that it is of the right type and is in suitable condition. Tubes bent or
weakened by corrosion and damaged couplers and planks/boards shall be
rejected.

 Photo 3 shows layout view for locations of Platforms A, B and C. Red zone
shows critical steep area while green zone is area that is still accessible.

 Photo 4 shows the anchoring that will be tied at anchor point with using
webbing sling, carabiner and kernmantle static rope. Loop of 8 will be
used at the anchor tie to carabiner and butterfly knot will be made at the
tie rope for hooking the harness.

 After all work activities that use the platform is done, the platform will be
dismantled by scaffolder in reverse sequence opposite to during the
installation.
 Sequence for the installation of the working platform can be referred to
photos as below:

9.2 SCAFFOLDING SUPERVISION AND INSPECTION

 A properly erected scaffolding consists of a firm foundation with properly


jointed standards and ledgers; adequate bracing with secure connections
between scaffolding and structure; safe working platforms; handrails and
safe access ladder.
 Scaffolding shall be erected, dismantled and modified by Scaffolders under
the supervision of a Scaffolding Supervisor. Other personnel such as helpers
or material handlers shall not be allowed to carry out scaffolding works e.g.
climbing, fastening or dismantling the scaffolding components or structures.
However, they are allowed to assist on handling of scaffolding materials
from the ground level only.
 Safety requirements prescribed in PTS 18.52.07 (Working at Height) shall
be complied with when working on a scaffold. This always includes the need
to use full body harness with double lanyards fastened to a suitable
anchorage point:

o during erection, modification, dismantling and inspection of scaffolding,


o when working at edge of a completed scaffolding where there is a risk of
fall from height e.g. due to strong wind conditions or instability due to
awkward body posture when performing tasks from the edge of the
platform.
o when a prohibition notice on restricted use of scaffolding is issued for
temporary removal of guardrails during removal or reinstatement of
plant equipment.

 Scaffolding Supervisor shall ensure that appropriate risk assessment such


as job hazard analysis is carried out prior to erection, dismantling and
modification of scaffolding and appropriate safety measures or controls are
identified and implemented. When carrying out risk assessment for
scaffolding, consideration shall be given to the following:

o Scope and location of work


o Access and egress to and from the ladder and working platform
o The position of the working platform

 All couplers shall be constructed of a structural material such as drop forged


steel, pressed steel and must be load bearing.
 Scaffolding planks used for toe boards and platform shall be fixed or tied
down.
 Joints in standards should be made with sleeve coupler and must always be
staggered.
 Ledgers shall be horizontal and clamped to the uprights using a load bearing
coupler.
 Joints in ledgers shall be made with sleeve couplers and must be staggered
so that no two joints shall occur in the same bay.
 Transoms shall be fixed to the ledgers using putlog / right angle couplers.
 All scaffolding must be properly braced to ensure stability. The scaffold
platform should be inspected before it is used for the first time and then

every 7 days, until it is removed. It should also be inspected each time it is


exposed to conditions likely to cause deterioration eg following adverse
weather conditions or following substantial alteration.

9.3 SCAFFOLDING MODIFICATIONS

 When the scaffolding erection has been completed, it is then the


responsibility of the Scaffolding Inspector to certify that it is correct and
safe for use. It shall not be modified in any way without reference to the
Scaffolding Supervisor. Any defects or unauthorized modifications should be
reported to the Scaffolding Supervisor.
 Any modification of scaffolding shall be carried out by the Scaffolder with
the supervision of the Scaffolding Supervisor.
 The Scaffolding Inspector shall, inspect and affix a scaffolding tag on the
scaffolding before it is put back into service.

o No changes/modifications shall be made on tagged (certified)


scaffolding while work is in progress using that scaffolding.
o If the scaffolding is found to be modified or unsafe to use at any time
after initial certification/tagging, the scaffolding tag shall be removed by
the Scaffolding Inspector.
o Notwithstanding above, the Scaffolding Supervisor should, on his own
or with assistance of other resources from the COMPANY, conduct an
independent or joint-inspection with the CONTRACTOR or party
responsible for the erection to check the compliance with appropriate
codes, standards or requirements on scaffolding.

 For temporary modification of scaffolding e.g. removal of guardrails or toe


boards due to operational reasons, access to the affected section shall be
controlled (e.g. via prohibition notice, barricade or warning signs). After
completion of the work, the scaffolding components and fittings shall be
reinstated as per original design before re-inspected and certified safe for
use.
 
9.4 SCAFFOLDING CONTROL PROCEDURE
 
 All scaffolding works e.g. erection, modifications and/or dismantling shall
obtain COMPANY approval prior to start of the work.
 During the erection of a scaffolding, warning notice should be affixed at all
access points e.g. ladder to indicate that the scaffolding is incomplete and
not safe to be used. All scaffolding shall be inspected by Scaffolding
Inspector before they are approved to be use. The appointed Scaffolding
Inspector shall complete and the scaffolding status tag which indicates the
location, inspection date, load rating (e.g. light duty, medium duty and
heavy duty), and a statement signify that the scaffolding is completed and
safe for use.

 In the event of any doubt concerning the integrity of a particular scaffolding,


Scaffolding Inspector or other person observing the situation should remove
the 'Scaffolding Complete, Safe for Use”' tags.
 All employees involve in the erection, inspection, modification, and/or use
of scaffolding should be made aware of this tagging system through safety
talks, meetings or training sessions.
 Scaffolding status tags should be valid for a limited and specific period of
time and should be re-validated by the Scaffolding Inspector before expiry.
 A system should be established for periodic inspection of scaffolding as they
may remain in position for many weeks. It is recommended that scaffolding
be inspected weekly. However, depending on rate of utilization and levels
of risk, specific inspection frequency may be specified by the COMPANY.
 In addition, they should be inspected after being exposed to severe weather
conditions e.g. heavy storms and rains which may affect the integrity and
stability of the scaffolding or ground. To guide the scaffolders including
scaffolding supervisor and inspector while the work is in progress and to
prevent incident (e.g. workers accidentally stepping onto
uncompleted/unsecured platforms), a system should be established to
demarcate sections which have been partially completed or not completed
using barricade, safety signs or tags.

9.5 SCAFFOLDING STORAGE AND MAINTENANCE

 Scaffolding planks/boards shall be stacked flat with adequate cribbing to


ensure that the bottom layer will not be in contact with water or moisture
on the ground. Cribbing should be inserted between each sixth layers of
planks/boards (approximately) to prevent accumulation of moisture.
 Any elevated area e.g. platform or structure designated for storage of
scaffolding materials shall be assessed or designed to withstand the load
i.e. weight of the storage including fittings. The scaffolding materials and
fittings shall be properly secured to avoid falling to the ground.
 Tubular scaffolding shall be stored flat. In all cases, cribbing shall be used.
 Scaffolding frames, braces and other parts shall be kept free of rust.

9.6 WORKING AT HEIGHT

 Scaffolders shall always wear full body harness complete with double
lanyards when erecting or dismantling scaffolds. They shall always clip on
when not working from a safe platform.
 Harnesses shall be secured to a suitable anchorage point as high as
possible above the working position to limit the height of any fall when in
use.
 All harnesses shall be inspected before each use for:

o Burns, cuts, or signs of chemical damage


o Loose or broken stitching
o Frayed web material

o D-ring and keeper pads are free from distortion and signs of undue
wear or damage
o Grommets and buckles are free of damage, distortion, or sharp edges
o
 The chest strap should be adjusted so that it is snug and located near the
middle of the chest. In a headfirst fall, a properly adjusted chest strap will
prevent the worker from coming out of the harness. Leg straps should be
adjusted so the user’s flat of the hand can fit snugly between strap and
leg.
 Harness straps should be adjusted to put the D-ring between the chest. A
properly positioned D ring will keep a worker stable during the work.
 The working platform shall be stable and of sufficient strength to support
all foreseeable loads, including workers, equipment and materials. They
shall also have adequate fall protection e.g. guardrails or railings on all
open sides, except where there is entrance to a ramp, opening, stairway
or ladder. The guardrails shall be provided with toe boards. When any part
of a barrier from any platform is removed, additional barriers shall be
fitted to prevent falls. Fall prevention rather than fall arresting (harnesses)
shall be the norm.
 In the case where any barrier has been removed or modified, a notice on
the restricted use of the platform may be required. Persons working on a
platform covered by a notice on the restricted use shall always wear a
harness with double lanyards and be tied-off at all time.
 Regular inspection and maintenance should also be carried out to ensure
that there is no risk of failure (from corrosion, fatigue) of any of the
components e.g. clips, bolts, guardrails.
 Scaffolds are designed to carry only people and small tools, hence should
be used according to the load they are designed for.

10.0 SAFETY PRECAUTIONS

 The work shall only be performed once received a valid PTW for the specific work.
 Safety precautions must be taken fully consideration before start and during the
activities whichever the emergency can be estimated to be happen.
 The JHA shall be clearly explained to all workers by the Work Leader/ Site
Supervisor during toolbox meeting. Work Leader/ Site Supervisor shall ensure all
personnel understand all the risks and hazards identified in the JHA.
 All personnel shall attend PTW Level 1 prior to enter site.
 Dedicated work leader shall attend and pass training for PTW Level 2.
 All personnel shall be made familiar with the Emergency Response Plan (ERP).
 All personnel shall be provided with and shall use the appropriate personal
protective equipment (PPE) e.g. safety helmet, safety glass, dust mask, safety
boots, hand glove, etc.
 Walkie-talkie shall be in good condition to ensure no communication failure.
 All tools and equipment shall be checked daily by qualified personnel prior to use.
 Any damaged equipment or tools shall not be used. They shall be tagged,
removed from site and replaced by equipment or tools in proper condition.
 A means of communication should by using walkie-talkie.
 CONTRACTOR should identify hazard pinch point and procedure to handle it.

 A Journey Management Plan (JMP) shall be setup and followed by all worker
during execution work.
 All vehicles associated with SSGP shall comply with Approved SSGP Land
Transport Procedure.
 CONTRACTOR to make sure housekeeping to be done upon work completion. All
the site debris need to be removed from site.
 Any emergency, workers must stop works and assemble to nearest “Assembly
Point”/ dedicated area.
 All personnel i.e. the Scaffolder, Scaffolding Supervisor and Scaffolding Inspector
shall use full body harness with double lanyard when working above 2.0m height
during erection, dismantling and modification of scaffolding. Safety requirements
for working at height as per PTS 18.52.07 (Working at Height) shall be complied
with.
 CONTRACTOR to ensure scaffolding erection, modification and dismantling
activities and the scaffolding structure itself shall not obstruct any emergency
access way or safety equipment, or cause damage to existing amenities such as
pipeline, piping and electrical/instrument equipment.
 For anchorage of scaffolding to fixed structure, the structure needs to be
approved by COMPANY supported by method statement. Never support
scaffolding with electric conduit, utility lines, process lines and equipment.
 CONTRACTOR to ensure all scaffolding planks/boards used must be kept well
away from heat sources such as furnaces, heat exchangers or hot piping to avoid
risk of damage to the items.
 Tools or equipment falling from height always have the potential to cause serious
injury or damage to facilities or equipment.
 In extreme weather conditions e.g. heavy rain, strong winds, ice, all work at
height shall be halted.
 CONTRACTOR to ensure all materials, tools and equipment shall be safely stored
and secured to prevent dislodgement in windy conditions or causing a trip hazard.
Material shall not be thrown up or down from any work area.
 CONTRACTOR shall ensure areas with possibility of falling object are clearly
indicated/barricaded to prohibit unauthorized persons from walking underneath.

1.0 INTRODUCTION

The following structural calculation covers the analysis and design for anchor life at scaffolding
platform with considered the anchor life load as presented in Appendix 1.0.

This calculation will cover 2 major conditions as below:

2.0 OBJECTIVES
The objective of this report is to design for scaffolding and stability to cater the entire imposed
load applied.

DESCRIPTIONS
Figure 1 defines available Protecta® First Harness Full Body Harness models and their key features. Harnesses are constructed in
the following styles (illustrated in Figure 2):
• Vest Style Full Body Harness

OPTIONS: Protecta Full Body Harnesses are available with a variety of options and accessories. The following is a partial list of
commonly used options and accessories (some options may not be available on all harnesses. See Figure 1):
• Shoulder D-rings
• Side D-rings
• Hip pad with side D-rings
• High visibility webbing

1.0 APPLICATIONS
1.1 PURPOSE: Full Body Harnesses are to be used as components in Personal Fall Protection System designed to prevent
a fall or safely arrest a fall. See Figure 1 for the harness models covered by this instruction and examples of their
application. Full Body Harnesses are used in the following applications:

Fall Arrest: Personal fall arrest systems typically include a Full Body Harness and a connecting
subsystem (energy absorbing lanyard). Maximum arresting force must not exceed 1,350 lbs (6
kN). For fall protection applications, connect the fall arrest subsystem (e.g.: lanyard, SRL, energy
absorber, etc.) to the D-ring or attachment element on your back, between your shoulder blades.

Work Positioning: Work positioning systems typically include a Full Body Harness, positioning
lanyard, and a back-up personal fall arrest system. For work positioning applications, connect
the work positioning subsystem (example: lanyard, Y-lanyard, etc.) to the lower (hip level) side
or belt mounted work positioning attachment anchorage elements (D-rings). Never use these
connection points for fall arrest.

Climbing: The Full Body Harness is used as a component of a climbing system to prevent the
user from falling when climbing a ladder or other climbing structure. Climbing systems typically
include a Full Body Harness, vertical cable or rail attached to the structure, and climbing sleeve.
For ladder climbing applications, harnesses equipped with a frontal D-ring in the sternal location
may be used for fall arrest on fixed ladder climbing systems.

Rescue: The Full Body Harness is used as a component of a rescue system. Rescue systems are
configured depending on the type of rescue. For limited access (confined space) applications,
harnesses equipped with D-rings on the shoulders may be used for entry and egress into confined
spaces where worker profile is an issue.

Controlled Descent: For controlled descent applications, harnesses equipped with a single
sternal level D-ring, one or two frontal mounted D-rings, or a pair of connectors originating below
the waist (such as a seat sling) may be used for connection to a descent or evacuation system

Restraint: The Full Body Harness is used as a component of a restraint system to prevent the
user from reaching a fall hazard. Restraint systems typically include a Full Body Harness and a
lanyard or restraint line.

CAUTION: Certain application and work conditions require the use of Full Body Harnesses with specific attributes:
• Full body harnesses with Kevlar web should be used when working with tools, materials, or environments of high temperature
(foundries, chemical manufacturing, steel fabrication, emergency rescue services, fire services, welders, oil industry, nuclear
industry, explosives).
• Harnesses with PVC coated hardware should be used when working in explosive or electrically conductive environments, or where
surfaces must be protected from the hardware.
• Harnesses with high visibility webbing should be used when increased visibility of the user is required.

1.2 STANDARDS: Harnesses included in this manual conform to the standard(s) identified on the front cover of this
instruction.
1.3 TRAINING: It is the responsibility of the user and the purchaser of this equipment to assure that they are familiar with
these instructions, trained in the correct care and use of, and are aware of the operating characteristics, application limits,
and the consequences of improper use of this equipment.

CAUTION: Training must be conducted without exposing the user to a fall hazard. Training should be repeated on a periodic basis.

1.4 RESCUE PLAN: When using this equipment and connecting subsystem(s), the employer must have a rescue plan and the
means at hand to implement and communicate that plan to users, authorized persons, and rescuers.
1.5 INSPECTION FREQUENCY: The Full Body Harness shall be inspected by the user before each use and, additionally, by a
competent person1 other than the user at intervals of no more than one year2. Inspection procedures are described in the
“Inspection and Maintenance Log” (Table 1). Results of each Competent Person inspection should be recorded on copies of
the “Inspection and Maintenance Log” (Table 1) or tracked with the i-Safe™ system (see “Inspection”).
2.0 LIMITATIONS AND REQUIREMENTS
Always consider the following application limitations before using this equipment:

1 Competent Person: One who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary,
hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.
2 Inspection Frequency: Extreme working conditions (harsh environments, prolonged use, etc.)may require increasing the frequency of competent person
inspections.

2.1 CAPACITY: The Full Body Harness is designed for use by persons with a combined weight (clothing, tools, etc.) of no
more than 220 lbs (100 kg)3. Make sure all of the components in your system are rated to a capacity appropriate to your
application
2.2 FREE FALL: Personal fall arrest systems used with this equipment must be rigged to limit the free fall to 6 feet (1.8 M).
Restraint systems must be rigged so that no vertical free fall is possible. Work positioning systems must be rigged so that
free fall is limited to 2 feet (.6 m) or less. Personnel riding systems must be rigged so that no vertical free fall is possible.
Climbing systems must be rigged so that free fall is limited to 18 in. (.46 cm) or less. Rescue systems must be rigged so
that no vertical free fall is possible. See subsystem manufacturer’s instructions for more information.

2.3 FALL CLEARANCE: There must be sufficient clearance below the user to arrest a fall before the user strikes the ground
or other obstruction. The clearance required is dependent on the following factors:

Figure 3 illustrates fall clearance calculation for a Shock Absorbing Lanyard. For other Fall Arrest subsystems, see
subsystem manufacturer’s instructions for fall clearance information.

2.4 SWING FALLS: Swing Falls occur when the anchorage point is not directly above the point where a fall occurs (see Figure
3). The force of striking an object in a swing fall may cause serious injury or death. Minimize swing falls by working as
close to the anchorage point as possible. Do not permit a swing fall if injury could occur. Swing falls will significantly
increase the clearance required when a self- retracting lifeline or other variable length connecting subsystem is used.

2.5 EXTENDED SUSPENSION: A Full Body Harness is not intended for use in extended suspension applications. If the user
is going to be suspended for an extended length of time it is recommended that some form of seat support be used.
DBI-SALA recommends a seat board, suspension workseat, seat sling, or a boatswain chair. Contact DBI-SALA for more
information on these items.

2.6 ENVIRONMENTAL HAZARDS: Use of this equipment in areas with environmental hazards may require additional
precautions to prevent injury to the user or damage to the equipment. Hazards may include, but are not limited to; heat,
chemicals, corrosive environments, high voltage power lines, gases, moving machinery, and sharp edges.

2.7 HARNESSES FOR HIGH TEMPERATURE ENVIRONMENTS: Harnesses with Kevlar webbing are designed for use in high
temperature environments, with limitations: Kevlar webbing begins to char at 800° to 900° Fahrenheit. Kevlar webbing
can withstand limited contact exposure to temperatures up to 1,000° F. Polyester webbing loses strength at 300° to
400° F. PVC coating on hardware has a melting point of approximately 350° F.

WARNING: When working with tools, materials, or in high temperature environments, ensure that associated fall protection
equipment can withstand high temperatures, or provide protection for those items.

WARNING: Although PVC coated, cadmium, or zinc plated hardware exhibit excellent corrosion resistance in chemical, acidic, alkaline,
and atmospheric conditions, frequent inspections may be required. Consult with Capital Safety if you question the use of this equipment
in hazardous environments.

2.8 COMPATIBILITY OF COMPONENTS: Capital Safety equipment is designed for use with Capital Safety approved
components and subsystems only. Substitutions or replacements made with non-approved components or subsystems
may jeopardize compatibility of equipment and may effect the safety and reliability of the complete system.

2.9 COMPATIBILITY OF CONNECTORS: Connectors are considered to be compatible with connecting elements when they
have been designed to work together in such a way that their sizes and shapes do not cause their gate mechanisms
to inadvertently open regardless of how they become oriented. Contact Capital Safety if you have any questions about
compatibility.

Connectors (hooks, carabiners, and D-rings) must be capable of supporting at least 5,000 lbs. (22.2 kN). Connectors
must be compatible with the anchorage or other system components. Do not use equipment that is not compatible.
Non-compatible connectors may unintentionally disengage (see Figure 4). Connectors must be compatible in size, shape,
and strength. If the connecting element to which a snap hook (shown) or carabiner attaches is undersized or irregular in
shape, a situation could occur where the connecting element applies a force to the gate of the snap hook or carabiner.
This force may cause the gate (of either a self-locking or a non-locking snap hook) to open, allowing the snap hook or
carabiner to disengage from the connecting point. Self- locking snap hooks and carabiners are required.

2.10 MAKING CONNECTIONS: Use only self-locking snap hooks and carabiners with this equipment. Use only connectors that
are suitable to each application. Ensure all connections are compatible in size, shape and strength. Do not use equipment
that is not compatible. Ensure all connectors are fully closed and locked.
Capital Safety connectors (snap hooks and carabiners) are designed to be used only as specified in each product’s
user’s instructions. See Figure 5 for inappropriate connections. Capital Safety snap hooks and carabiners should not be
connected:
A. To a D-ring to which another connector is attached.
B. In a manner that would result in a load on the gate.
CAUTION: Large throat snap hooks should not be connected to standard size D-rings or similar objects which will result in a load
on the gate if the hook or D-ring twists or rotates, unless the snap hook complies with ANSI Z359.1-2007 and is equipped with a
3,600 lb (16 kN) gate. Check the marking on your snap hook to verify that it is appropriate for your application.
C. In a false engagement, where features that protrude from the snap hook or carabiner catch on the anchor, and
without visual confirmation seems to be fully engaged to the anchor point.
D. To each other.
E. Directly to webbing or rope lanyard or tie-back (unless the manufacturer’s instructions for both the lanyard and
connector specifically allows such a connection).
F. To any object which is shaped or dimensioned such that the snap hook or carabiner will not close and lock, or that
roll-out could occur.
G. In a manner that does not allow the connector to align properly while under load.

2.11 CONNECTING SUBSYSTEMS: Connecting subsystems (self-retracting lifeline, lanyard, rope grab and lifeline, cable
sleeve, etc.) must be suitable for your application (See section 1.1). See the subsystem manufacturer’s instructions for
additional information. Some harness models have web loop connection points. Do not use snap hooks to connect to web
loops. Use a self-locking carabiner to connect to a web loop. Ensure the carabiner cannot cross-gate load (load against the
gate rather than along the backbone of the carabiner). Some lanyards are designed to choke onto a web loop to provide
a compatible connection. Lanyards may be sewn directly to the web loop forming a permanent connection. Do not make
multiple connections onto one web loop, unless choking two lanyards onto a properly sized web loop. To choke the lanyard
on a web loop:

2.12 ANCHORAGE STRENGTH: Anchorages selected for use with the Full Body Harness must have a strength capable of
sustaining the static load requirements of the intended fall protection application:
• Fall Arrest: Anchorages selected for Fall Arrest must sustain loads of 3,372 lbs (15 kN) or greater.
• Work Positioning: Anchorages selected for Work Positioning must sustain 1,012 lbs (4.5 kN) or greater.
• Climbing: The structure to which a climbing system is attached mustsustain the loads required by that particular
system. Consult the system manufacturer’s product instructions for requirements.
• Rescue: Anchorages selected for Rescue systems must sustain loads of 1,012 lbs (4.5 kN) or greater.
• Controlled Descent: Anchorages selected for Controlled Descent must sustain loads of 3,373 lbs (15 kN) or greater.
• Restraint: Anchorages selected for Restraint must sustain loads of 450 lbs (2 kN) or greater.

3.0 DONNING AND USE

DANGER: Do not alter or intentionally misuse this equipment. Consult Capital Safety when using this equipment in combination with
components or subsystems other than those described in this manual. Some subsystem and component combinations may interfere with the
operation of this equipment. Use caution when using this equipment around moving machinery, electrical and chemical hazards, and sharp
edges.

WARNING: Consult your doctor if there is reason to doubt your fitness to safely absorb the shock from a fall arrest. Age and fitness seriously
affect a worker’s ability to withstand falls. Pregnant women or minors must not use any Protecta full body harness.

3.1 BEFORE EACH USE of this equipment inspect it according to “Inspection and Maintenance Log” (Table 1).

3.2 PLAN your system before use. Consider all factors that will affect your safety during use of this equipment. The following

• ANCHORAGE: Select an anchorage that meets the requirements specified in “Limitations and Requirements”.
• SHARP EDGES: Avoid working where system components may be in contact with, or abrade against, unprotected
sharp edges.
• AFTER A FALL: Components which have been subjected to the forces of arresting a fall must be removed from
service and destroyed.
• RESCUE: The employer must have a rescue plan when using this equipment. The employer must have the ability to
perform a rescue quickly and safely.

3.3 DONNING AND FITTING THE PROTECTA FULL BODY HARNESS: If your harness incorporates loops for a removable
waist belt, the belt should be installed through the belt loops and side D-ring slots as shown in Figure 8. The hip pad, if

3.4 USE OF FALL ARREST D-RING OR ATTACHMENT ELEMENT: For fall arrest applications connect to the D-ring or
attachment element on your back, between your shoulder blades. Side D-rings, if present, are for positioning or restraint
applications only. Shoulder retrieval D-rings are for rescue or retrieval applications only. The front D-ring is for ladder
climbing or positioning. D-rings on a seat sling are for suspension or positioning applications only. (See Section 1.1.)

3.5 MAKING CONNECTIONS: When using a hook to connect to an anchorage or when coupling components of the system
together, ensure roll-out cannot occur. Roll-out occurs when interference between the hook and mating connector causes
the hook gate to unintentionally open and release. Self-locking snap hooks and carabiners should be used to reduce the
possibility of roll-out. Do not use hooks or connectors that will not completely close over the attachment object. See
subsystem manufacturer’s instructions for more information on making connections.

3.6 CONNECTING SYSTEM COMPONENTS: After fitting the full body harness the user may then connect to other system
components. Follow the guidelines in Section 2 and the manufacturer’s instructions included with the component.

4.0 INSPECTION

WARNING: If the full body harness has been subjected to fall arrest or impact forces it must be immediately removed from service
and destroyed.

4.1 INSPECTION FREQUENCY: The First Full Body Harness must be inspected at the intervals defined in Section 1.5.
Inspection procedures are described in the “Inspection and Maintenance Log” (Table 1).
4.2 DEFECTS: If inspection reveals a defective condition, remove unit from service immediately and destroy.
4.3 PRODUCT LIFE: The functional life of Protecta harnesses is determined by work conditions and maintenance. As long as
the product passes inspection criteria, it may remain in service.

NOTE: Only DBI-SALA or parties authorized in writing may make repairs to this equipment.

5.0 MAINTENANCE, SERVICING, STORAGE


5.1 WASHING INSTRUCTIONS:
Full body harness: Clean full body harness with water and a mild soap solution. Do not use bleach or bleach solutions.
Wipe off hardware with a clean, dry cloth, and hang to air dry. Do not force dry with heat. An excessive buildup of dirt,
paint, etc. may prevent the full body harness from working properly, and in severe cases degrade the webbing to a point
where it weakens and should be removed from service. More information on cleaning is available from Capital Safety.
If you have questions concerning the condition of your harness, or have any doubt about putting it into service contact
Capital Safety.

5.2 AUTHORIZED SERVICE: Additional maintenance and servicing procedures must be completed by a factory authorized
service center. Authorization must be in writing. Do not attempt to disassemble the unit.

5.3 STORAGE: Store full body harnesses in a cool, dry, clean environment out of direct sunlight. Avoid areas where chemical
vapors may exist. Thoroughly inspect the full body harness after extended storage.

6.0 SPECIFICATIONS

Harness Hardware Inspect harness hardware including buckles (1), D-rings (2), back pad (3),
(Diagram 1) loop keepers (4), etc. These items must not be damaged, broken, distorted,
and must be free of sharp edges, burrs, cracks, worn parts, or corrosion. PVC
coated hardware must be free of cuts, rips, tears, holes, etc. in the coating to
ensure non-conductivity. Ensure buckles work smoothly.
Webbing & Stitching Inspect webbing; material must be free of frayed, cut, or broken fibers. Check
(Diagram 2) for tears, abrasions, mold, burns, or discoloration. Inspect stitching; Check for
pulled or cut stitches. Broken stitches may be an indication that the harness
has been impact loaded and must be removed from service.

Labels All labels should be present and fully legible. See Figure 9.

Warranty to End User: D B Industries, LLC dba CAPITAL SAFETY USA (“CAPITAL SAFETY”)
warrants to the original end user (“End User”) that its products are free from defects in materials and
workmanship under normal use and service. This warranty extends for the lifetime of the product
from the date the product is purchased by the End User, in new and unused condition, from a CAPITAL
SAFETY authorized distributor. CAPITAL SAFETY’S entire liability to End User and End User’s exclusive
remedy under this warranty is limited to the repair or replacement in kind of any defective product
