Project Title: Proposed Two (2)-Storey Residential Building

Project Location: 312 M.H. Del Pilar Street, Maysilo, Malabon City

1. That I am the owner of the said project.

2. That the said development/construction on the property will be undertaken by my

administration. In House, this means that I will not hire the services of any Constructor.

3. That I will take all charges on y employee during the construction of the said project.

4. That I am executing this Affidavit to affirm and/or confirm the foregoing and for whatever
legal intents and purposes this may serve.
