RELEASE, WAIVER & QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

That I, ILUMINADA LAGONERO BIAZON with a registered address at

POROK 2 BALUBAL 4322 SARIAYA QUEZON PHILIPPINES hereby
acknowledge that I have received the amount of ONE HUNDRED FIFTY
THREE AND THIRTY SEVEN CENTAVOS (₱153.37) Philippine
currency, as full settlement of my claim arising out of and in connection
with the inability to access and use involving my GCash Account
Number 09309371252 on MAY 11, 2023.
In consideration of the foregoing:
1. It is understood that my GCash account underwent validation in
relation to the Enhanced Due Diligence Procedure pursuant to
the Money Laundering and Terrorism Prevention Program and
the same was undertaken with due notice and without malice
on the part of GXchange, Inc., (GXI).
2. The validation of my account may take longer than expected and
I may not be able to use my funds therein and therefore I
requested GXI to just release the same and close my said
account. GXI has therefore released the ONE HUNDRED FIFTY
THREE AND THIRTY SEVEN CENTAVOS (₱153.37) out of its
outstanding customer relations and I hereby release and forever
discharge GXI and all its officers and employees from any and
all claims and causes of action of any kind or nature whatsoever
concerning the amount mentioned above;
3. I acknowledge that I have received all amounts that are now due
me from GXI;
4. Accordingly, I hereby close my said GCash Account and hereby
request GCash to close the same;
5. I will not at any time in any manner whatsoever directly or
indirectly disclose to any person or entity any information of any
kind relating to or arising out of my claim subject matter of this
quitclaim, otherwise I undertake to pay GXI the liquidated
damages equal to the amount (₱807.55).
6. I acknowledge that I and my children have no cause of action
whatsoever against GXI, its officers, directors, employees in
respect of any matter incident to or arising out of my claim
which was settled by GXI, to my full and complete satisfaction.
I further warrant that I will institute no action and I will not
continue to prosecute pending action, if any, against GXI,
7. Nothing herein shall be taken by my heirs or assigns as a
confession and/or admission or the liability on the part of GXI,
its officers, agents or employees for any matter, cause or
demand or damages I and my children may have against any or
all of them, of which I and my children have none.
I finally declare that I have read and understood this document and
the Release, Waiver and Quitclaim hereby given are made willingly, freely
and voluntarily and with full knowledge of my rights under the law.
