Instruction for filling up the Fresh Application for Online registration u/s 19 of RCI Act, 1992: 
   
1. Please read the instructions mentioned on the form carefully before filling up the form.   
2.  Fields  indicated  with  ‘*’  mark  are  mandatory.  Appropriate  information  should  be  filled  by  the 
candidates.   
3. The candidate's name should be filled in the online application form as per the name mentioned in 
his/her class Xth / XIIth examination certificate issued by the respective Examination Board.  
4. Before Filling up the Online Application form following documents must be kept ready:  
   Scanned Colour copy of the following documents of size below 200kb in JPG / Pdf format to be 
uploaded : 
‐   Recent Passport size colour photograph (JPG ) 
‐   Certificate of Authentication with recent passport size colour photograph (JPG / Pdf) 
‐   Original Final year Mark sheet of Rehabilitation Qualification   (JPG / Pdf) 
‐   Original Rehabilitation Qualification Certificate(JPG / Pdf) 
‐   Xth / XIIth Certificate / Marksheet (for age proof & name) (JPG / Pdf) 

5.  Payment of fees of Rs.1000/‐ online through payment gateway. 
6. Download the Certificate of Authentication format from your dashboard.  Original Certificate of 
Authentication on institutes letter head with recent colour passport size photograph, name, sign & 
stamp of the institution from where the candidate has passed out the RCI approved rehabilitation 
qualification should be scanned.   
7. References of 2 Persons with name, address and contact number to be filled. 
8.  Status of Registration can be checked on your dashboard.   
9. Provisional Online Application ID must be used as reference for any further enquiries related to 
registration.  
10.  e‐  Registration  Certificate  with  permanent  CRR  number  will  be  issued  online  only  after 
verification of scanned documents submitted by the applicant. If any information is found fake / 
distorted/ fabricated / manipulated / forged / tailored the application will stand cancelled.  
11. All the documents to be attached must be scanned with original documents.
